What is Neomycin / dexamethasone (Otomize)?

Otomize is an ear spray combining an antibiotic (neomycin), a corticosteroid (dexamethasone) and acetic acid, treating the infection and the inflammation of the ear canal together. The spray format doses evenly without drops needing to find their own way down.

The condition it treats

Otitis externa is inflammation of the outer ear canal, the itchy, painful, sometimes discharging ear common in swimmers and cotton-bud users. Left untreated it can swell the canal shut; treated topically it usually settles within a week.

How the service runs on Scripter

Patients book online, complete the pre-consultation questionnaire, and the guided pathway walks the practitioner through the otitis externa consultation with the screening and counselling steps built in. The supply decision, the record and any GP notification are produced in the same flow, documented against the exact protocol version in force.

Training and eligibility requirements.

Who can deliver this service, and who it can be delivered to. The governing PGD document defines the definitive inclusion, exclusion and supply criteria; the Scripter consultation pathway enforces them, and every consultation is documented against the protocol version it ran under.

Practitioner training
✓Registered healthcare professional from a profession authorised to supply under PGDs (Schedule 16, Part 4 of the Human Medicines Regulations 2012), such as a pharmacist, nurse, paramedic or physiotherapist, with current registration with their professional regulator
✓Practising from premises suitable for the service, such as registered pharmacy premises or a registered clinic
✓Assessed clinical competency for this service, declared and reviewed as part of PGD authorisation
✓Sign-off against the current version of this PGD in Scripter before first supply; sign-offs expire automatically when the protocol is updated, and the platform notifies everyone affected
✓In-date basic life support and safeguarding training
✓Indemnity arrangements covering the service
Patient eligibility
✓Patients within the governing PGD's age range with otitis externa confirmed by otoscopic examination
✓Perforated eardrum or grommets excluded before supply; neomycin is ototoxic beyond an intact drum
✓Systemic illness, spreading cellulitis or suspected malignant otitis externa referred urgently
✓Water precautions and cotton-bud counselling completed in the pathway

Neomycin / dexamethasone (Otomize) PGD questions

Why does the practitioner look in the ear first?+

Two reasons: to confirm the diagnosis is outer-ear rather than middle-ear, and to check the eardrum is intact, because neomycin must not be used through a perforation. Otoscopy is a required step in this pathway.

How fast does Otomize work?+

Discomfort usually eases within 48 to 72 hours, with the full course completed over about a week; failure to improve triggers the pathway's referral step.

Can swimmers keep swimming?+

Keeping the ear dry during treatment matters; the counselling step covers water precautions and how to prevent recurrence afterwards.
