What is Clarithromycin?

Clarithromycin is the macrolide alternative for acute otitis media when the patient is allergic to penicillin. The PGD uses the same otoscopy-led assessment as the amoxicillin pathway and reserves antibiotics for the presentations national guidance says will benefit.

The condition it treats

Acute otitis media is a middle ear infection, common in young children, causing earache, fever and sometimes a bulging or perforated eardrum. Most cases settle without antibiotics; a defined minority benefit from treatment.

How the service runs on Scripter

Patients book online, complete the pre-consultation questionnaire, and the guided pathway walks the practitioner through the acute otitis media (penicillin allergy) consultation with the screening and counselling steps built in. The supply decision, the record and any GP notification are produced in the same flow, documented against the exact protocol version in force.

Training and eligibility requirements.

Who can deliver this service, and who it can be delivered to. The governing PGD document defines the definitive inclusion, exclusion and supply criteria; the Scripter consultation pathway enforces them, and every consultation is documented against the protocol version it ran under.

Clarithromycin PGD questions

When is clarithromycin used instead of amoxicillin?+

Only with a documented penicillin allergy; otherwise amoxicillin remains first line.

Are there important interactions?+

Yes. Clarithromycin interacts with several common medicines including some statins and anticoagulants; the consultation checks the medication list before supply.

How quickly should the ear improve?+

Pain usually eases within two to three days; the safety-net advice explains when to seek review sooner.
